AN ACCIDENTAL DEATH BENEFIT PAYOUT FOR THE FIRST TWO CERTIFICATE YEARS.
Death from natural causes during the first two years of coverage pays just a portion of the benefit amount. During this period, your beneficiary receives an amount equal to 110% of the premiums paid. Full benefits will be paid for accidental death from first day of coverage. After two years, full benefits are payable for any covered loss.
-
As a CSEA Affiliate member, you’ll pay your premiums through the convenience of automatic payroll deduction. If you retire, your premiums will be deducted from CalPERS.
-
If you change your mind, you can return your Certificate of Insurance within 30 days, without claim, and get a full refund of any premium paid – no obligation, no questions asked.
-
You can’t be singled out for a rate increase due to changes in your health. Your rates will change when you enter a new five-year age band (e.g., 35-39, 40-44, 45-49, etc.). New York Life can only increase premiums on a class-wide basis. For example, a class is a group of people with the same age or gender.
-
Yes. This coverage is renewable until you reach age 80. The benefit amount reduces by 50% at age 70. Premiums do not reduce.
-
You may select any person, persons, trust or other legal entity as your beneficiary. You may change your beneficiary at any time by written request. Your beneficiary is the person(s) last designated by you in writing and recorded by or on behalf of New York Life Insurance Company. If, at the time of your death, there are no surviving beneficiaries, benefits will be paid to the executor or administrator of your estate, or at the option of New York Life Insurance Company, to the surviving relatives in the following order of survival: spouse; children equally; parents equally; or brothers and sisters equally. You are the automatic beneficiary for dependent insurance, as described in the Certificate of Insurance.
-
Insurance will take effect on the date your application is approved by New York Life Insurance Company, provided the initial contribution has been paid.
-
Insurance will end on the earliest of:
- • The last day of the insurance period during which you are no longer a member of the association
- • The last day of the insurance period during which you reach age 80
- • The last day of the 60-day grace period that follows the end of the insurance period for which the last contribution has been paid
- • The last day of the insurance period during which your retirement allowance from a California Public Employees Retirement System ceases or becomes insufficient for contribution deductions
- • For an insured spouse, the last day of the insurance period during which such insured spouse ceases to be your lawful, married spouse or domestic partner
- • The last day of the insurance period following receipt of your request to end the insurance
- • The day before the day the policy ends or is changed to end insurance for the group of insureds to which you belong

There is a limited benefit: Death from natural causes during the first two years of coverage pays just a portion of the benefit amount. A conversion right is available if insurance ends for any reason except non-payment of premiums or you are requesting to end insurance. However, if insurance ends because the policy terminates or changes for the group of insureds to which you belong, a conversion right is only available if you have been continuously insured under the policy for at least five years. Consult your Certificate of Insurance for complete details.
